Empowering individuals & communities
through Knowledge, connection,
AND prevention.
Rooted in Wellness Education (RIW) is a community-based nonprofit dedicated to bridging the gap between clinical care and community health. We provide essential support and education for people of all ages who are living with—or seeking to prevent—chronic conditions such as diabetes, obesity, cancer, and cardiovascular disease.
Our programs focus on prevention and empowerment, equipping individuals with practical, evidence-based tools to take charge of their health. From personalized nutrition guidance and therapeutic cooking classes to stress management, movement, and peer-to-peer support, we help people build lifelong habits that support recovery, reduce the risk of recurrence, and promote long-term wellness.
At RIW, we believe wellness should be accessible, culturally sensitive, and rooted in dignity. By removing barriers and meeting people where they are, we empower individuals to transform their health—and their communities.
We don’t just teach wellness—we cultivate it. 🌱

Our Mission is to cultivate a healthier, more sustainable community where individuals and families can thrive across generations. We expand access to nutritious food, provide practical nutrition education, and strengthen connections—self, family, peer, and community—to build a lasting foundation of health and resilience.
Our Vision is to cultivate a future where every individual—regardless of geography or economy has the knowledge, resources, and support to live a vibrant, healthy life. Through access to nutrition education and holistic prevention, RIW aims to reduce the burden of chronic diseases like diabetes, obesity, cancer, and cardiovascular illness, transforming communities through empowerment, equity, and lifelong well-being.
Our Commitment is to meet people where they are, with dignity and cultural sensitivity, while removing barriers that stand in the way of lifelong wellness.
"Rooted in compassion, driven by purpose—because love never fails."

Bridging The Gap Between
Clinical Care & Community Health.

Time Constraints in Healthcare
Healthcare providers deliver essential care, but appointments are often short — sometimes only 10–15 minutes — making it hard to cover nutrition, stress management, or daily lifestyle habits.
Patients’ Experience
You may leave visits with important questions about applying medical guidance to your meals, routines, and mental wellbeing.
The Gap
Clinical care provides the “what” — Rooted in Wellness helps
with the “how,” offering practical support to translate medical
advice into attainable, life-long habits. We help navigate
what starts in the clinic—like a diagnosis or
recommendation—into lasting change at home, in the
kitchen, and in the community.
How Rooted in Wellness Supports Patients
A Partner, Not a Replacement
We work alongside healthcare providers to extend their guidance:
“While doctors provide the diagnosis and treatment plan, Rooted in Wellness gives patients the time, tools, and personalized support to put those recommendations into practice.”
Real-Life Context
From grocery shopping to meal prep and stress-reduction strategies, we help families implement healthy changes in everyday life.
Support Where It’s Needed Most
Because doctor visits can’t always cover every concern, we’re here to give patients space, tools, and encouragement to take steady steps toward lasting wellness.
